The Center for Rehabilitation at Memorial Hospital is an inpatient rehabilitation facility (IRF) designed for patients who are recovering from a recent illness, stroke, injury or disorder that has affected their functional ability. The interdisciplinary rehabilitation program is needed to address the complexity of the patient’s medical, nursing and rehab needs. Patients will participate in an intensive therapy program at least three hours per day, at least five days per week. Examples of conditions treated include:
  • Stroke.
  • Spinal cord injury.
  • Amputation.
  • Major multiple trauma.
  • Brain injury.
  • Neurological disorders (multiple sclerosis, Parkinson’s disease, Guillain–Barré, motor neuron disorders, polyneuropathies, muscular dystrophy).
  • Hip fractures.

Individualized Care Plans

Our interdisciplinary team will assess each patient and determine what the anticipated interventions, functional outcomes and discharge plan will be. Each plan is then discussed at the family meeting with the rehabilitation physician, continuing care nurse, registered nurse, caregiver and patient.  

Stroke Support Group

To help you and your family or caregivers cope with the emotional aspects of your illness or injury, a Stroke Support Group is held twice a month in the Center for Rehabilitation. Led by a registered nurse and a neuropsychologist, the group meets on the second Wednesday of each month at 12 p.m. and the fourth Tuesday of each month at 4 p.m. Our staff will assist you and your caregivers with referrals to community support groups dealing with other specific diagnoses.
