As a new patient in the Family Care practice, here are a few things we suggest you do:1. Notify your HMO
If you are a member of a Health
Maintenance Organization (HMO), it is important to notify your plan of the name of your primary care physician in the Family Care practice.2. Transfer your medical records
Please ask other healthcare providers you have seen to forward your complete
medical records to us. This will help us to better care for you.
Requests for medical record transfers must be made in writing. For your
convenience, if needed, we will provide you with forms to make this process easier for you. Contact Medical Records at (401) 729-22303. Choose a primary care physician
You have chosen the Family Care Center as your primary care practice. You can choose a primary care physician for yourself and for each family member. As family physicians, the doctors emphasize taking care of entire families, but if you wish, you may choose a different doctor for each member of your family. Your doctor will be directly responsible for your healthcare by providing or arranging for any care you may need.
