Please schedule an appointment with us if you are due for a routine physical or have other health-related concerns. Office hours are Monday, Tuesday, Thursday, and Friday from 9:00 a.m. to 5:00 p.m. and Wednesday from 9:00 a.m. to 8:00 p.m. Please call during office hours to schedule an appointment.

For your convenience, open access scheduling is available. Based on same day scheduling, we use a system that makes it easier for you to be seen when it is most convenient for you.
You can schedule your appointment on the same day as long as you call between the hours of 9:00 a.m. and 3:00 p.m. and up to five working days in advance. This is for all types of visits – follow ups, acute visits or physicals.
If you are sick and there are no available appointments, please ask to speak to the nurse on your team.
School and camp physicals
When scheduling school or camp physicals, be sure that you have transferred all immunization records from your previous physician. These records are necessary for completing all school and college forms and most camp and preschool forms.
Special ArrangementsChanging appointments
If you need to cancel or reschedule an appointment, it is important that you call us at least 24 hours before your appointment, so that we may schedule another patient who may be waiting to see the doctor.
If you need a special appointment time due to transportation or work schedules, please discuss this with our staff.
After hours
Family Care always has a family physician on call. If you need urgent medical care after hours, on weekends or holidays, call your practice.
Emergencies
Life-threatening emergencies
demand immediate attention. In emergencies such as possible heart attack, stroke, excessive bleeding or loss of consciousness, call 911 or your local emergency phone number, or go directly to the nearest medical facility - Memorial Hospital if possible.

available on site at no charge.
