DIRECTIONS FOR COMPLETING THIS ORIENTATION PROGRAM

1. Please read all of the following information contained in the Student and Faculty Hospital Guide and the Annual Mandatory Education Booklet below.
2. Print out the QUESTIONAIRE and Confidentiality Agreement.
3. Answer the QUESTIONAIRE (answers contained in the Annual Mandatory Education Booklet).
4. Bring the completed QUESTIONAIRE and signed Confidentiality Agreement with you on your first day at Memorial Hospital to be given to Professional Development or your Department Head.
