Services:Memorial's outpatient physical therapy program is designed for patients of all ages. Professional staff at our three locations conduct individualized, objective assessments of neurological, orthopaedic and medical conditions. Your evaluation focuses on your clinical history and measures of functional deficits. Once your assessment is complete, a treatment plan is tailored to meet your needs and goals for restoring strength and improving mobility.
We emphasize education and your return to work or sports. Treatment plans incorporate various techniques in conjunction with a home exercise program.
A written referral from your physician is required for therapy. It must include your diagnosis, the physician's orders, and be dated no more than 30 days before your initial visit to us.
