Job Description:
The RN - Nurse Care Manager develops plans and implements coordination of services for patients in collaboration with the multidisciplinary health care team.  He/She will work with healthcare team with the full implementation of the patient centered medical home model within a primary care physician's office.  He/She will be required to work with both nursing and physician staff to facilitate practice to help achieve positive patient outcomes.  Responsibilities include
(1)Perform comprehensive screening assessments
(2)Care Coordination
(3)Disease education
(4)Self-management support to adult patients with chronic health conditions like CAD, Depression, Diabetes, and monitors compliance
(5)Actively participates in multidisciplinary patient centered team meetings
6)Develops quarterly reports
(7)Attends required training and collaborative sessions

HOURS:  Non-consistent day time schedule applies.  Start times may vary, evening hours as needed.  Part Time:  40 hours per 2 wk/pay period
Grant Position.

EDUCATION:  Graduate of NLN accredited School of Nursing.  BSN required.
EXPERIENCE:  Primary Care Practice, Community Nursing experience required.
LICENSE:  Current Rhode Island  RN License
KNOWLEDGE:  Identify and implement components of the nursing process, identify learning needs and teach patients and families; communicate, collaborate, and delegate effectively; work independently to achieve goals.  Organizational, priority, problem solving, and decision making skills required; ability to lift and move patients, ability to sit for extended period of time, ability to travel by car.  Self-motivation, commitment to continuing education and professional accountability.  Knowledge of community services, support agencies, government regulations and proficiency with computer skills including Electronic Medical Records.

Special Qualifications:  Bilingual in Spanish and/or Portuguese preferred.  CDOE Certificaiton within 1 year of employment required.  Case Management Certification preferred.
