Job Description: The Manager of Continuing Care oversees the discharge needs of MHRI patients working collaborativelly with the entire healthcare team. He/She assures all patients are assessed on admission and a discharge plan is developed and implemented. The manager is also responsible fro establishing standards of practice for the discharge planning team, insuring staff competence, continuously evaluating and improving effectiveness of the process and systems which support patient care and staff performance. He/She covers staff for vacancies and increased census and complies with all regulatory requirements.

Department:  Continuing Care
Education:  Graduate of NLN accredited School of Nursing; BSN required and Masters degree preferred.

Licensure: Rhode Island RN license required; certification in Utilization Review preferred.

Experience: Prior acute care clinical and management experience required.  Prior experience in monitoring utilization and measuring outcomes of patient care required. 

Knowledge: Demonstrated clinical competence with minimum three years' relevant clinical experience required. Knowledge and ability to interpret reimbursement, licensing and other regulatory/accrediting standards. Excellent skills in communication, collaboration and negotiation. The Performance Improvement Coordinator supports continuous regulatory readiness compliance activities for TJC, CMS, and RIDOH.  These activities include organization, implementation, and evaluation of mock tracer surveys hospital wide and specialty areas; development and implementation of written action plans in response to mock and/or actual survey findings; monitoring of Measures of Success to ensure resolution of findings/issues within assigned timeframe.  Supports hospital-wide initiatives comprising best practices for patient satisfaction, externally required projects (HARI, QIO, RIDHO, BCBS, CMS, Leapfrog).  Will assess hospital-wide systems to suport the reduction of health care errors and improves patient safety. Provides education regarding CQI concepts and tools to PI Team members, Service Chiefs, Department Managers, Hospital Executives, Physicians and other clinical staff.


Hours:  8:00am - 4:30pm
Education:  BSN or CPHQ certification required.
Experience:  Hospital quality improvement, health care education, Joint Commission survey preparation and coordination, team traiing and a minimum of 2 years Medical/Surgical experience is required.

In addition, the Professional Development coordinator is responsible for initial and ongoing assessment of Clinical Services staff competence.The Professional Development Coordinator is accountable for the dissemination of information regarding MHRI, The Joint Commission and other regulatory standards to all Clinical Services departments, and collaborates with department managers regarding methodologies to ensure compliance with these standards.Acting as a role model for staff, the Professional Development Coordinator utilizes current research and performance improvement outcomes in the development and implementation of education which affects patient care delivery.
