Job Description: Major Responsiblities:
The Nurse Practitioner is assigned clinical responsibility to provide health and medical care for a specified patient population within a legally defined scope of practice and in collaboration with a physician consultant.  He/she uses established standards of care to develop, implement, and evaluate an integrated plan of care designed to promote, maintain, and/or restore health.  He/she is accountable for directing care and patient education related to health maintenance and illness prevention and for the monitoring of patient outcomes.  It is expected that the Nurse Practitioner actively seek opportunities to improve processes and systems within his/her practice arena and function in the roles of provider, mentor, educator, researcher, and consultant.

Utilizes skills in planning, organizing, and priority setting
Ability to identify learning needs and teach patients and families
Ability to communicate, collaborate with other disciplines, and delegate effectively
Knowledge of community services, support agencies, and government regulations
Multilingual skills highly desirable.

Location:  Primary Care Centers of Plainville, Mass
Hours:  PART TIME    8:30am - 5:00pm    Monday - Friday.
Education:  Graduate of NLN accredites School of Nursing and Masters level NP preparation required.
Experience:  1-2 years experience in a Family Medicine setting required.
License:  Board Certified Family Medicine Nurse Pratitioner with current license as RN and NP to practice in Massachusetts.  Prescriptive privileges in Massachusetts.
